Download and print our Service Contract.  Written in plain English, it sets forth the basic terms and conditions for our coverage.  Complete the remittance page and mail it along with your check for the desired coverage to: OhioCCDS, 6475 Perimeter Drive #113, Dublin, OH 43016.

Once we receive the completed Remittance Page along with your payment, we will immediately process it and mail your signed contract back to you along with our Welcome Package.  Your coverage begins on the date payment is received.
